Clinical Innovation Group (CIG) – Open Invite to GPs

The LMC have asked us to remind practices of the purpose of the monthly CIG meeting and the opportunity for gps to attend and\or comment on papers.   

The purpose of the Countywide Clinical Innovation Group is to both bring together the clinical resource of the three CCGs in Worcestershire to shape the clinical strategy and direction and focus on clinical innovation; and to lead and advise on service development and changes to commissioning policy ensuring improvements in patient experience, clinical outcomes, efficiency and value for money. It  provides a forum for clinical challenge and debate and a focus for joint working, coherence & co-ordination of service development with clear accountability and responsibility. The Group has a clinical advisory function and will make recommendations to Clinical Executive Committee and Financial Recovery Board and other committees as required.

Key Responsibilities

  • Consider service developments, issues and strategies and provide clinical challenge and input with a view to forming a consensus of clinical opinion.
  • Provide a forum for in-depth discussion and the development of recommendations for Clinical Executive concerning CCG priorities, including recommendations about areas not to be pursued.
  • To provide clinical advice, guidance and recommendations to Clinical Executive  Committee and to Financial Recovery Board.

The next meeting takes place on Thursday 8th August, 2pm at Barnsley Hall, Bromsgrove. The agenda and papers can be found here.

First 5 Network Co-design event Saturday 14th September 2019

We are planning to develop a First 5 Network that responds to the needs of early career GPs & we need your help!

Next Generation GP programme bookings open

Following on from the initial expression of interest request in June, we are really pleased to announce that the Next Generation GP programme is coming to Herefordshire and Worcestershire from September 2019. The programme consists of one evening session per month for five months.  More information is detailed in the attached letter including speakers. The ‘Next Generation GP’ is a programme for emerging leaders in general practice designed by, and for, a group of early-career GPs and trainees. Our aim is to inspire GP trainees and newly qualified GPs to be informed leaders as well as excellent clinicians, with the insight and connections they need to change and improve the system in which we work.

QOF End of Life QI Model  

As part of the 2019-20 QOF Framework there are two QI modules – one of which is in relation to End of Life Care.

Practices may have experience in QI methodology and hence have a plan and have begun to implement changes. Others may have less experience and may value some support in developing a QI module that suits the practice and has quantifiable impact for patients.

The Herefordshire and Worcestershire CCGs are proposing to offer a half-day workshop to support practices to develop their ideas and give some hints and tips on QI implementation and some thoughts on areas of focus. The workshop will be run by:

  • Dr Maggie Keeble, Care Home GP & ICOPE (Integrated Care for Older people) Clinical Lead; and
  • Dr Clare Marley, Clinical Lead & Joint Senior Responsible Officer for End of Life Care for Herefordshire & Worcestershire STP

Please share this with your EOL clinical lead and let us know if interested – if there is sufficient interest we will endeavour to put a date in the diary for the end of September. Backfill for attendance of 1 GP from each practice is funded via Promoting Clinical Excellence in Worcestershire and is also included in the proposed changes to HOF. Other members of the practice team are also welcome to attend.

QI003: The contractor can demonstrate continuous quality improvement activity focused on end of life care as specified in the QOF guidance (27 points)

QI004: The contractor has participated in network activity to regularly share and discuss learning from quality improvement activity as specified in the QOF guidance. This would usually include participating in a minimum of two network peer review meetings. (10 Points)

The document ‘2019/20 GMS Contract QOF framework’ states in relation to the QI modules:

“The aim of this domain is to provide support for contractors and their staff to recognise areas of care which require improvement, and take steps to address this through the development and implementation of a quality improvement plan and sharing of learning across their network. Being skilled in quality improvement has been recognised as a key role for healthcare professionals in the Shared View of Quality129.

NHS England and GPC England have worked with the Royal College of General Practitioners, NICE and the Health Foundation to develop the topic specific guidance included here. This guidance sets specific objectives for each topic which contractors are expected to work towards and provides advice on potential quality improvement actions. Within the parameters set out in this guidance, contractors are encouraged to understand where they have the potential to make quality improvements and then to design and implement bespoke quality improvement plans, including improvement targets to address these. There are no deadlines given for the completion of the diagnostic activities, the subsequent plan or the network meetings. However, contractors are advised that they are expected to be working on these improvement activities throughout the QOF year.

The two topic areas identified for 2019/20 are prescribing safety and end of life care.”

NHS Pension Scheme

Proposed flexibility launched by Department of Health & Social Care

The NHS Pension Scheme is a highly valuable part of the package of pay, terms and conditions for NHS staff, which continues to compare very favourably with schemes in other sectors. It provides hard working and dedicated staff with security in retirement after decades of public service and patient care.

However, the relative generosity of the scheme means that for some staff, largely senior doctors, changes since 2010 to the way that wider pensions taxation works are causing significant financial concerns to those doctors and is causing many to look closely at whether it is in their financial interest to do extra work for the NHS. For some, the potential impact of the changes are causing them to consider retirement or withdrawal from the pension scheme.

The DoH proposals aim to give senior NHS clinicians more control over pensions growth, so they can continue to provide the services that patients need.

This consultation asks for your views on proposals, including a 50:50 option to halve pension accrual and contributions.


Referral Form Update

WRH Nuclear Medicine - New Gamma Camera

Update July 2019

'The nuclear medicine gamma camera has been successfully replaced and the new scanner is now operational.

There will be ongoing staff training  and familiarisation with the new system but the department is now clinically operational again.

This week's Prescribing Top Tip is regarding Treatment of Mild Dry Eye Conditions in Primary Care

Delays to Cervical Screen Results

There are currently delays in people receiving the results from their cervical screen as part of the NHS Cervical Screening Programme. Letters are usually issued within 14 days of the original sample-taking appointment; however, people across England are currently experiencing longer waits due to ongoing changes within the programme. While the level of delay is unlikely to cause or increase the risk of clinical harm, we need to ensure that we manage expectations so as not to cause unnecessary worry.

Key messages for sample takers:

  • NHS England and NHS Improvement has committed to the full implementation of HPV Primary Screening across England by December 2019. This is due to the increased sensitivity of the test, longer protection of a negative result and better outcomes. 
  • Please ensure that people are advised at the time of their cervical screening appointment that there may be a delay in receiving results.   Please remind people that the delay in receiving their result is unlikely to cause or increase the risk of clinical harm to them. It will also have no implications for any subsequent treatment that is required.
  • Whilst it is very unlikely that the level of delay will cause or increase the risk of clinical harm, please advise people that if they experience significant delays and are worried, to contact their GP practice or the clinical where their sample was taken. Sample takers may contact Primary Care Services England on behalf of patients for any queries relating to screening results, using the following link https://pcse.england.nhs.uk/contact-us/.
  • It is expected that the delays in people receiving the results of their screen will continue until HPV Primary Screening is fully implemented across England.

Cervical Cytology Administration Changes 1 August 2019

From 1 August 2019, the administration service that supports the national cervical screening programme will transfer to the NHS. The service produces and sends out the cervical screening invitation, reminder and result letters each year to women. From 1 August 2019, the service will be managed by the NHS North of England Commissioning Support Unit (NECS).  Commissioning Support Units are part of the NHS and provide a range of services to NHS organisations locally and nationally. The new service will be called NHS Cervical Screening Administration Services – please see branding below.

While women who receive cervical screening invitations, reminder and result letters should see no change, the transfer will mean some practical changes to the administration of the service and this briefing explains some of those changes.

  • Online forms will be hosted on a new NECS website at www.csas.nhs.uk. Web content will transfer, including forms such as cease/defer/reinstate. End users will see no difference to these forms other than a change in branding (see above) and instructions about contact points.
  • Any submissions to PCSE right up to the point of change will be processed.
  • After 1 August 2019, any email sent to previous PCSE inboxes will have clear redirect information for service users. Similar messages on the PCSE website and cervical screening messages will have redirect links to NECS new website at www.csas.nhs.uk. This will remain in place for a period of time. 
  • The PO Box will change from 1 August for paper correspondence, new details are as follows: PO Box 572, Darlington DL1 9AG.
  • During the transition, telephone contact will continue to be routed through the PCSE Customer Support Centre and the number (0333 014 2884) will remain in place.

In the meantime, please continue to use existing PCSE contact points for requests and queries about the current service.

Primary Care Awards – entries by 30 August

The National Association of Primary Care (NAPC) is now inviting entries for its 2019 Awards. They recognise people, teams and organisations who have provided outstanding patient care through innovation, integration, or simply dedication to their job. Physicians Associate Preceptorship Programme Opportunity

An opportunity has arisen within the PA Preceptorship Programme to host a Physician’s Associate. There is currently a PA who needs to be linked up with a practice, ideally this would be at a practice in the north of the county. 

The PAs in the programme are newly qualified and this will be their first role, they will spend 5 sessions per week in general practice and 4 sessions in the acute trust on medical wards. This will enable them to bring transferable skills from their learning at the acute into general practice. One session every fortnight will be spent in a post graduate training programme bring all the PAs together to allow them to follow a general practice followed curriculum delivered by local consultants. This also gives them an opportunity to network and share their learning.

The trust will act as the host employer with a secondment agreement to participating practices. The practice will receive a £5,000 bursary to be used as they feel is necessary to support the PA in their first year.

GP Mentoring Available for all GPs

What does GP Mentoring offer?

We are proud to announce a free mentoring service that is available for all GPs in Herefordshire and Worcestershire, including newly qualified GPs. The mentoring team can offer mentoring, support and signposting.

Who can use this service?

The service can be used by any GP who would like to explore ways to develop themselves. This could be personally, professionally or within their career. The aim is to provide mentors to support GPs in fulfilling their potential, whether this is in practice or in other leadership positions.

Early Career GPs

GP Mentors can offer support to help you:

  • Gain practical & academic experience in clinical leadership from local experts in the field
  • Build a network of expert contacts with medical and NHS management leaders
  • Enhance the skills & competencies that are essential for future primary care leaders
  • Develop competencies relevant to the domains defined in the developing regional Leadership Model
  • Develop & deliver a specialty or management driven project relevant to specific needs of Primary Care
  • Access a postgraduate qualification, for example, in Leadership or Medical Education

GP Mentoring Flyer 

Free childcare for parents of 3 and 4-year olds

We are trying to improve take-up by parents of their early years childcare entitlement. It has no impact on family income but enables the young child to attend an Ofsted registered setting (Childminder, nursery, holiday club etc.) for up to 15 hours per week free. There are other ways to reduce the cost of childcare and these are mentioned in the poster, and explained on the website, childcarechoices.gov.uk

The Department for Education have supplied copies of the attached posters and we propose sending these to GP practices and Health Visitor clinic over the next few days for display.

We are looking to expand our existing medical secretarial support in order to provide the best possible service to our patients.

We are seeking an experienced medical secretary who is highly organised, confident and self-motivated and who has the ability to work effectively as part of the team. Good communication skills (both written and oral) are essential, as is the ability to work independently within a busy office.

As this is a job-share role, the successful candidate will be expected to work with our existing medical secretary to provide a seamless support service to the GPs. He / she will also require a level of flexibility and be willing and able to cover periods of annual leave.

An AMSPAR qualification in medical administration is desirable, as is knowledge of EMIS Web
