This weekly Member Practice Update is produced by NHS Redditch and Bromsgrove, NHS South Worcestershire and NHS Wyre Forest Clinical Commissioning Groups for circulation in your practice, to provide you with essential guidance, information and useful support.

Herefordshire and Worcestershire CCG Nominations and Election Process – Governing Body GP Locality Leads

In anticipation of the proposed merger being approved and a new NHS Herefordshire and Worcestershire CCG being created on 1st April 2020, member practices are advised that an election process for GP Leads will be taking place during September and October. This will comprise of four roles as follows:

  • 1 x Herefordshire Governing Body GP Locality Lead
  • 1 x Redditch and Bromsgrove Governing Body GP Locality Lead
  • 1 x South Worcestershire Governing Body GP Locality Lead
  • 1 x Wyre Forest Governing Body GP Locality Lead


Information relating to the nominations and election process is accessible here, along with the planned timeline to complete the process for these roles. As is usually the case, the results of this election process will be independently overseen by the Local Medical Committee.

Full job descriptions outlining the requirements of the role and nomination forms will be circulated to the relevant practices on Monday 2nd September, in line with the planned timeline.

The PCE contract has funded a GP from each Worcestershire practice to attend this event.  We would also like to extend the invite for the morning sessions to include your practice ICT lead.

The event is also open to Herefordshire GPs and IT leads.

GP and Hospital/Healthcare provider Portfolio Working opportunity – Expressions of interest sought

We are pleased to share a further two Portfolio role opportunities with you on behalf of Worcestershire Acute Hospitals NHS Trust & Worcestershire Health and Care Trust. The closing date for expressions of interest for these roles will be Friday 6th September.  

A detailed Job Plan for the opportunities can be found within the attachment. Please review and complete the Expression of Interest form if you are interested in applying.

Social Prescribing Guidance for PCNs

Please find two recent documents released by NHSE with regards to Social Prescribing. The Reference guide and Technical Annex provides additional information to help PCNs introduce the role of social prescribing link workers into their multi-disciplinary teams.

The guidance also recognises that PCNs are at different stages with social prescribing and has highlighted that this first year will be important for all partners to learn from emerging practice, to build in feedback and review processes, and to hear from a wider range of perspectives in local areas and opportunities and challenges. The STP as an Integrated Care System is also expected to have an agreed STP-wide plan for Social Prescribing. Therefore, to enable this, the CCGs, and Public Health in both counties will be working with PCNs and partners to develop an STP-wide Social Prescribing network for PCNs and their partners, to include exploring the potential of expanding the existing Social Prescribers Network, too. More information on the first meeting of this STP-wide network will follow from Steve Connelly, the CCG’s lead for Social Prescribing.

New contract for GP clinical IT systems

The current GP Systems of Choice (GPSoC) contract, under which the core clinical systems (EMIS, TPP SystmOne, Vision and Microtest) and other subsidiary systems and services are provided to all GP practices, comes to an end in December 2019.

Severe Mental Illness Physical Health Checks Local Incentive Scheme - Update

Following feedback from practices, some amendments have been made to the EMIS template for recording Physical Health Checks for patients on Severe Mental Illness (SMI) registers. These has been done to make some mandatory requirements clearer, to resolve some difficulties where completion of QRISK was inappropriate either due to age or an existing CVD diagnosis, and to future proof the template by ensuring it is fully SNOMED compliant.

Practices are asked to please re-upload the revised template locally so that it can be used for all future health check recording. Where practices have used the old template and experienced difficulties around particular sections, these will be addressed directly with practices on an individual basis once claims are received, to ensure there are no difficulties with payments.

Specialist Pharmacy Services have produced a written instruction which can be adopted by healthcare organisations to enable employees to receive the seasonal influenza vaccine. More details can be found on the SPS webpage

Dermatology Referral Guidelines

In line with the 19/20 PCE Contract, Primary Care are asked to view and utilise the revised referral guidelines for patients presenting with Dermatological conditions.

Whilst the guidelines do not provide an exhaustive list or override clinical judgement, they are intended to ensure that patients are directed to the most appropriate provider, maximising the community offer and reducing demand on Worcestershire Acute Services, thus helping to reduce waiting times. 

Early Help Pathway

'Dear Early Help colleague

As you are aware the Multi Agency Levels of need guidance for Worcestershire was updated in  May 2019. Following feedback from Early  Help colleagues, please find the Early Help Pathway attached.

The Early Help Pathway details the approach professionals in Worcestershire use to respond to the emerging needs of the children, young people and families they are working with. This Pathway is intended as guidance to assist in ensuring the right help is provided at the right time.

This Early Help Pathway can be used in conjunction with the Multi Agency Levels of need guidance to help support children, young people and families in Worcestershire document of which it is part of.

Personalised care for veterans in England

NHS England and NHS Improvement, together with the Ministry of Defence, have published the Personalised care for veterans in England, a guide for clinical commissioning groups and local authorities, which sets out a new personalised care approach for those veterans who have a long term physical, mental or neurological health condition or disability. 

This guide is for those individuals and organisations who are leading or involved in supporting this patient group through the delivery of NHS Continuing Health Care or a jointly agreed care plan. 

A supporting patient leaflet has also been published. Both documents are attached for information, along with a supporting brief.

In Alliance August 2019 

Welcome to the August 2019 edition of ‘In Alliance’, a newsletter about the work and activity of the three Worcestershire Alliance Boards (Redditch and Bromsgrove, South Worcestershire and Wyre Forest).

The goal of the Alliance Boards is to remove boundaries at the point that care is delivered, to create a system where patient interests come first, and resources are collectively focused on improving health outcomes, supporting people to stay well and to live independently for as long as they wish. Via the Alliance, providers work together to deliver new models of care that is focused on local people and located in natural neighbourhoods.

The work of the Alliance Boards is achieved through Neighbourhood Teams, of which there are 14 across the county. Neighbourhood Teams bring together a team of professionals who provide care and support to the registered population of a group of GP practices.

Safeguarding Compliance 

In response to the Annual Regulatory Reviews with practices, the CQC has advised that there seem to be different views about the levels of safeguarding training expected.

CQC has advised that it will be expecting practices to comply with the guidance which is given in Nigel’s Surgery #25 and #33. The weblink is below:


Both of these documents were updated in July 2019 to reflect updated guidance.

IG Primary Care Training Times Newsletter

'Good Afternoon All,

As the Midlands and Lancashire CSU are responsible for providing the IG Primary Care Training across the Hereford and Worcester region, I would be extremely grateful if you could arrange for the attached ‘Training Times’ newsletter to be circulated to all GP practices within your CCG areas. It is important that we get as many people as possible attending these sessions given Information Governance is a mandatory training requirement that must be updated annually to ensure compliance. This is also one of the mandatory requirements of the Data Security and Protection Toolkit, so to ensure compliance we need to get the key members from each practice attending one of the sessions listed on the flyer.

THC Warning 

Since the start of the year there have been a dozen or so reports of incidents in schools and amongst school age children in Bury, Rochdale and Oldham involving substances that were sold as either ‘THC vape’/‘THC vape pens’/‘THC oil’ or ‘cannabis oil’. The reports usually indicate the substance was vaped in an electronic-cigarette device. Some of these incidents have led to pupils collapsing and to ambulances being called. 

Therefore please be mindful that this practice will not be isolated in the North west but also nationally.

First 5 Update

A message from Dr Mobeen Ahmed, First 5 Clinical Lead in Herefordshire and Worcestershire

I would like to introduce myself as the designated Clinical Lead for First 5 GPs in our STP.  

I was appointed earlier this year and I am passionate about raising the profile of our local first 5 GPs and helping them come together for mutual benefit and to allow sustainable general practice in our area.

I live and work in Hagley (located in the Wyre Forest area of Worcestershire) and feel strongly that our aspirations, career development and opportunities could be supported in a central way. The overall aim is to allow young GPs to feel welcome and valued so that going forwards the needs of our patients can continue to be met.

We have a tremendous pool of resource within our STP but often there is a perceived lack cohesion and shared opportunities for networking and learning. We are planning to have mentoring from experienced GPs, large scale events and also with time small group sessions. We have some high profile speakers planned to attend some of our key events, such as the Next Generation GP Programme.

To allow us to promote events, discussion and support for the First 5 cohort I would be grateful if those GPs who have completed their GP training within the last 5 years to email their details to the Primary Care Workforce Team sowoccg.workforce@nhs.net . Next Generation GP programme bookings open

Following on from the initial expression of interest request in June, we are really pleased to announce that the Next Generation GP programme is coming to Herefordshire and Worcestershire from September 2019. The programme consists of one evening session per month for five months.  More information is detailed in the letter including speakers. The ‘Next Generation GP’ is a programme for emerging leaders in general practice designed by, and for, a group of early-career GPs and trainees. Our aim is to inspire GP trainees and newly qualified GPs to be informed leaders as well as excellent clinicians, with the insight and connections they need to change and improve the system in which we work.

GPFV Clinical Lead Update 

A message from Dr Meeraj Shah, GPFV Workforce Clinical Lead- Herefordshire and Worcestershire

Hello everyone.

I am a GP Partner and Trainer at Elgar House surgery in Redditch. I have recently been appointed as the GPFV Workforce Clinical Lead for Herefordshire and Worcestershire. I will be working 2 sessions a week to help deliver the workforce plan for the STP area.

Over the next few years, it is anticipated that a number of experienced clinicians will be retiring. This will potentially leave us with a gap in the primary care workforce within the STP area. Our aim is to get more clinicians at different stages of their careers to work within Worcestershire and Herefordshire. To achieve this, we are developing a number of exciting projects.

We have set up a First Five Network for GPs who have completed their training in the last 5 years. We have invited a number of high profile speakers such as Dr Nikki Kanani MBE  to attend events organised through the Next Generation GP programme. Experienced GPs can be an invaluable source of information and guidance to younger GPs. We have set up a Mentorship programme to achieve this.

We are in the process of setting up an education academy and also encouraging more GP practices in becoming training practices.

Training Opportunities from the RCGP

As we continue to support development of the workforce within Herefordshire and Worcestershire, we are working to compile and centralise training opportunities within the area. While we streamline this process, we will publish these sessions within the MPU to keep you up to date with what is available. This information will also be included on the Training and Events page on our intranet.  

End of Life Workshop

If you have not yet done so and would like to express an interest to attend this workshop (details below),  please email the Primary Care generic mailbox: sowoccg.worcestershireprimarycare@nhs.net if you are would find this helpful and would like to attend.

As part of the 2019-20 QOF Framework there are two QI modules – one of which is in relation to End of Life Care

Practices may have experience in QI methodology and hence have a plan and have begun to implement changes. Others may have less experience and may value some support in developing a QI module that suits the practice and has quantifiable impact for patients.

The Herefordshire and Worcestershire CCGs are proposing to offer a half-day workshop to support practices to develop their ideas and give some hints and tips on QI implementation and some thoughts on areas of focus. The workshop will be run by:

  • Dr Maggie Keeble, Care Home GP & ICOPE (Integrated Care for Older people) Clinical Lead; and
  • Dr Clare Marley, Clinical Lead & Joint Senior Responsible Officer for End of Life Care for Herefordshire & Worcestershire STP

Please share this with your EOL clinical lead and let us know if interested – if there is sufficient interest we will endeavour to put a date in the diary for the end of September.  Backfill for attendance of 1 GP from each practice is funded via Promoting Clinical Excellence in Worcestershire and is also included in the proposed changes to HOF.  Other members of the practice team are also welcome to attend.

QI003: The contractor can demonstrate continuous quality improvement activity focused on end of life care as specified in the QOF guidance (27 points)

QI004: The contractor has participated in network activity to regularly share and discuss learning from quality improvement activity as specified in the QOF guidance. This would usually include participating in a minimum of two network peer review meetings. (10 Points)

The document ‘2019/20 GMS Contract QOF framework’ states in relation to the QI modules:

“The aim of this domain is to provide support for contractors and their staff to recognise areas of care which require improvement, and take steps to address this through the development and implementation of a quality improvement plan and sharing of learning across their network.  Being skilled in quality improvement has been recognised as a key role for healthcare professionals in the Shared View of Quality129.

NHS England and GPC England have worked with the Royal College of General Practitioners, NICE and the Health Foundation to develop the topic specific guidance included here. This guidance sets specific objectives for each topic which contractors are expected to work towards and provides advice on potential quality improvement actions. Within the parameters set out in this guidance, contractors are encouraged to understand where they have the potential to make quality improvements and then to design and implement bespoke quality improvement plans, including improvement targets to address these. There are no deadlines given for the completion of the diagnostic activities, the subsequent plan or the network meetings. However, contractors are advised that they are expected to be working on these improvement activities throughout the QOF year.

The two topic areas identified for 2019/20 are prescribing safety and end of life care.”

GP Patient Survey July 2019 release

The table shows the performance of the 83 GP Practices in Herefordshire and Worcestershire on 6 specific questions from the GP Patient Survey released in July 2019 which covered the periods January to March 2019. These 6 questions were identified in a national article after the January 2016 GP patient survey release and used to rank GP practices nationally.

GP Mentoring –  The service can be used by any GP who would like to explore ways to develop themselves. This could be personally, professionally or within their career. The aim is to provide mentors to support GPs in fulfilling their potential, whether this is in practice or in other leadership positions. GP Support Team –The CCGs and LMC have a well established GP Support Team in place, whose main focus is to respond to, and empower practices to make operational and team changes that directly and indirectly address challenges they are facing in the short to medium term. The GP Support Team can be accessed via the LMC or Sally Everton sally.everton@nhs.net at the CCGs. 

WORCECSTER CITY ONLY – Teledermatology Pilot – Rashes only

  • Teledermatology pilot will commence in September as soon as training has been provided, running for three months.
  • DMC will provide training to practices in the first two weeks of September – dates have been suggested and will be confirmed ASAP.
  • GPs are asked to download the DMCTelederm app in anticipation of the training, nhs.net email addresses will be required for registration.
  • All GPs are asked to refer patients via Teledermatology as and when appropriate, ensuring patients meet the specific criteria (details on new referral forms – to be added to EMIS).
  • For queries relating to the Teledermatology app, please direct queries to: Lauren Hampton, DMC - Assistant Community Services Manager (
  • The CCG would like to capture views of all participating GPs at the end of the pilot and therefore a short questionnaire will be circulated upon completion.
  • All Teledermatology referrals should be coded appropriately on EMIS (using code H8lo).

An evaluation will be undertaken at the end of the pilot, results will be reviewed and shared with pilot practices.

Assuming the pilot is successful, the intention is to roll Teledermatology out to all practices.
